Working Together for a Healthy Community

The Thumb Community Health Partnership (TCHP) is located in the rural area of Michigan referred to as the Thumb. The partnership encompasses Huron, Lapeer, Sanilac, and Tuscola County.  Members include public mental health, health departments, local hospitals, human service agencies and community agencies.

Community Health Worker (CHW) Partnership Program

Thumb Community Health Partnership (TCHP) is seeking partner agencies to help implement a regional Community Health Worker (CHW) Program focused on improving chronic disease outcomes, expanding community health navigation, and building sustainable CHW services across Huron, Lapeer, Sanilac, and Tuscola counties. Selected organizations may receive up to $20,000 per CHW through performance-based funding to support training, navigation services, chronic disease programming, and billing capacity development.
