The Thumb Community Health Partnership collects and reviews data that allow the partners to understand the strengths of the community, health protective factors,
and health risks experienced by residents.
TCHP Dashboard
The partnership created a dashboard to help monitor progress including the growth of the network, system changes, health outcome indicators, and program impact. Dashboard Data is broken down in the tabs above.
Review of data enables partners to create solutions to build a healthier community. In 2019, through a planning grant from the Office of Rural Health Policy, the partnership prioritized Behavioral Health and Obesity Related Chronic Disease for focus and action.

2023 Thumb Region Community Health Needs Assessment
This comprehensive Community Health Needs Assessment(CHNA) adopts a holistic "Whole Person Approach" to evaluate and address the health needs within our community, emphasizing the pivotal roles of behavioral health, chronic illness, and social determinants of health in influencing the quality of life across all stages of life
2023 Chronic Disease Listening Session
Michigan Department of Health and Human Services (MDHHS)partnered with local organizations in each of Michigan’s ten prosperity regions to hold listening sessions with community members who have lived experience with unmet social needs and chronic disease. MDHHS will use these sessions to guide the development of a Social Determinants of Health Accelerator Plan for Michigan.
This assessment was conducted in order to better understand the behavioral health needs of the region and to promote an integrated approach to meeting those needs. This report will serve as a launch pad for important discussions and making decisions about solutions-- solutions which recognize a healthy body and mind are critical to overall wellness.
