Path (Personal Action Toward Health) Program

An interactive six-week workshop designed to support individuals who face the daily challenges of living with chronic conditions. PATH programs are for individuals who live with conditions such as arthritis, heart disease, diabetes, asthma, depression, and other diseases.  Learn about self-management strategies and how you can help better manage your health!
